Support Planning

Planning for the future

We believe all young people have the potential to find their own solutions given the right support and resources.

Through collaboration between staff and the young people we support, we use support plans to deliver person-centred planning. This is a tool used by health and social and care professionals that measures and supports the change and enables positive outcomes.

These outcomes can be anything from building confidence, getting a job, maintaining independence, or better management of certain behaviours.

What will be in my Support Plan?

  • Your needs and requirements.
  • How these needs will be met.
  • Documentation on how this plan can facilitate your needs and wishes.
  • How our support will help you do the things you want to do.
  • Document and details of your finances and personal budget.
  • A plan to prepare you for the transition to adulthood.

Floating Support

Planning for the future

Warwickshire Living wants to spread our message of hope and compassion. We believe that a single action can make a difference in the community, and that collective action can greatly impact the world. Through advocacy and outreach activities, our team works tirelessly each day to contribute their part to the greater good. We provide floating support and community based support services for young people who may not have a tenancy with but leaving in their own homes or within a community-based setting where they can benefit from peer support.

Supporting challenging circumstances
These include supporting people to maintain tenancies by better managing challenging circumstances such as family breakdowns or mental ill-health. Usually these services are for short periods of time until the person feels they no longer require support. A combination of one-to-one support and peer support, through workshops and drop-ins, empower people to take matters into their own hands by realising their abilities again or for the first time. Through these services, people will be encouraged to participate in social or group activities such as joining walking, gardening, exercise and coffee clubs.
Warwickshire Living Ltd are committed to providing a safe and secure environment for young people aged 16 - 24 helping them prepare for adulthood.
